The Health Network la Côte (RSLC) is one of the 4 networks of care in Vaud according to the Cantonal Law on Care Networks (LRS). It brings together care providers (hospitals, EMS, CMS, doctors, etc.) and municipalities at the regional level. The mission of the network is to improve, in close collaboration with its members, the care of users within the system X. The RSLC manages the Regional Bureau (BRIO) whose main missions are: assisting individuals losing autonomy upon discharge from the hospital, management and requests for accommodation in EMS X, and complementing its social offers with its partners “Hospitals” and “medical practices”. The Health Network la Côte is looking for a social worker at % in CDI.Your main activities:At the acute care hospitals of the RSLC:• Ensure the processing of social and insurance requests that need to be addressed during in collaboration with caregivers (including liaison services).• If necessary, refer individuals to community social services.In the medical practices of the RSLC:• At the request of the doctor, ensure the processing (initiating actions and/or referral to other professionals) of social and insurance requests that need to be addressed for adults whose access to care is compromised by social and/or financial difficulties (housing, lack of insurance, specific knowledge, and/or support for example). • Be a resource person for social issues for hospital and medical practice services.Profile:- Degree in social work (Bachelor of Arts HES-SO, social service) or an equivalent qualification.- CAS specialist in professional analysis in the field of social action, or addictions – an asset.- Minimum of 2 years of experience in the health sector (hospital, extra-hospital and/or EMS and/or medical-social institution), as well as in network work.- Very good knowledge of social insurance, the Vaud medical-social system, and procedures in relation to the Justice of Peace.- Strong writing skills and proficiency in standard tools.- Proven ability to work in a multidisciplinary team and in a network.- Proactivity in their area of expertise.- Good listening skills and a vehicle with a driver's license.Offer:- A pleasant working environment within a dynamic team;- Social conditions according to the CCT of the Vaud public health sector;- Opportunities for continuing education.- Start date: as soon as possible.
