IMAD is a pioneering and leading institution in home care in the canton of Geneva. It provides care, support, prevention, and health promotion services 7 days a week, 24 hours a day, to more than 18,000 patients of all ages, 25% of whom are under 65 years old.

With its 2,300 expert and committed collaborators, including more than 50 teams deployed throughout the canton, IMAD's mission is to meet the population's needs and their wish to remain at home autonomously while preserving their quality of life during periods of temporary or prolonged fragility. It also helps to limit hospital stays, facilitate return home, and relieve informal caregivers.

ARCADE Liaison Nurse / 80% / Permanent Contract



The ARCADE liaison nurse (IDL) is responsible for informing, guiding, and recording requests from the public, particularly patients, future patients, and informal caregivers, to ensure the quality of care management. The IMAD arcade, located in the Gustave Julliard Building of the HUG, is attached to the liaison service with a presence 7 days a week.

The ARCADE IDL works closely with liaison nurses from the various liaison sites and covers their activities during weekends and public holidays. The specific scope of the liaison nurse covers hospitalised patients requiring a return home with nursing care. Their activities include assessing needs, medically validating required services, and coordinating planned interventions by home care providers.

Your missions and responsibilities

  • Inform the public and HUG staff about home care services offered by IMAD, partners in the Geneva social-health network, tariffs, and reimbursement procedures for home care services. When necessary, direct the patient to the appropriate person or structure for advice
  • Handle telephone responses for the Proch’infos line
  • Coordinate between IMAD and the hospital sector to ensure follow-up of hospitalised IMAD patients
  • Coordinate between the home care provider and the hospital sector to organise the return home of patients requiring nursing care
  • Manage the administrative registration of new IMAD patients
  • Promote the exchange of useful and necessary information for patient care (according to the provided documentation)
  • Facilitate hospital discharges and promote a return home under the best conditions
  • Contribute to health promotion and prevention by understanding the issues and links with the Geneva social-health network
  • Collaborate with the patient, informal caregiver, health and social professionals
  • Prevent inappropriate (re)hospitalisations
  • Work closely with the IMAD arcade team located at the HUG.

Qualifications

  • Bachelor HES-SO in nursing or equivalent training recognised and registered by the Swiss Red Cross, authorisation to practise in the canton of Geneva
  • Knowledge of care processes and clinical pathways, Geneva health-social network
  • Ability to work independently and interprofessionally within a multidisciplinary team
  • Sense of responsibility and solution orientation, communication, negotiation, and interpersonal skills
  • Adaptability and delegation skills
  • Stress and conflict management.
