The Réseau Santé la Côte (RSLC) is one of the 4 Vaudois care networks according to the Cantonal Law on Care Networks (LRS). It brings together care providers (hospitals, nursing homes, home care centers, doctors, etc.) and municipalities at the regional level. The mission of the network is to improve, in close collaboration with its members, the quality of care and the user experience in the health system. The RSLC manages the Regional Office and Orientation Bureau (BRIO) whose main missions are: supporting people losing autonomy upon hospital discharge, managing and coordinating requests for accommodation in nursing homes, and providing medico-social support at the hospital and home levels.To complete its social offer to its Hospital and Medical Practice partners, the Réseau Santé la Côte is looking for:One social worker at [percentage]% or 2 social workers at 50% on a permanent contract.Your main activities:At the RSLC acute care hospitals:- Ensure the processing of social and insurance-related requests that must be handled during hospitalization in collaboration with caregivers (including liaison services).- If necessary, direct the patient to competent community social services.At the RSLC medical practices:- Upon the doctor's request, ensure the processing (initiation of procedures and/or referral to other professionals) of social and insurance-related requests that must be handled for adults whose benefit from care is compromised by social and/or financial difficulties (housing, lack of insurance, lack of specific knowledge and/or support, for example).- Be a resource person for social issues for hospital services and medical practices.Profile:- Degree in social work (Bachelor of Arts HES-SO, social work) or equivalent title.- CAS as a specialist in the analysis of professional practices in the field of social action, or addictions – an asset.- Experience of at least 2 years in the health sector (hospital, extra-hospital and/or nursing home and/or medico-social institution experience), as well as in network work.- Very good knowledge of social insurance, the Vaudois medico-social system, and procedures in connection with the Justice of the Peace.- Writing skills and mastery of standard tools.- Proven ability to work in a multidisciplinary team and in a network.- Proactivity in the field of competence.- Listening skills and empathy.- Vehicle and driver's license required.We offer:- A pleasant working environment within a dynamic team.- Social conditions according to the CCT of the Vaudois parapublic health sector.- Possibilities for continuing education.Start date: as soon as possible.Complete application to be sent by March 20.Additional information: BRIO. jid7c20812aen jit0518aen jpiy26aen
