We are looking for a specialized nurse for the medical-social centers (CMS) of Moudon and Payerne, for a 60% - 80% role. Mission: Ensure monitoring of the client’s health evolution regarding mental health and/or at home, and manage their care project by organizing interventions on an interdisciplinary basis and in collaboration with the client, their family, and their doctor. Perform mixed tasks, encompassing general nursing: - Hold a recognized nurse diploma by the Swiss Red Cross or an equivalent title deemed appropriate, preferably with specific experience in mental health, and ideally have at least 2 years of professional experience in general and elderly care. - Be able to work autonomously with an interdisciplinary approach. - Have reflective skills with analysis and synthesis abilities, along with a good command of and management of care, being comfortable in dealing with various stakeholders. - Hold a valid driver’s license and possess a vehicle (possibility to use a professional vehicle).
