It offers a flexible work environment that promotes balance between professional and private life and conditions encouraging the development of skills as well as the sharing of knowledge and experience.Liaison Nurse / 100% / 7-month fixed-term contractThe scope of the liaison nurse covers IMAD clients (known or future clients) who are hospitalized and require a return home with IMAD services. Their scope of activity is broken down in particular in terms of needs assessment, medical validation of the required services, and forecasting of interventions by home care teams (EMD) and/or centralized services.Missions and responsibilities- Inform clients or future clients of the home care services offered by IMAD, partners of the Geneva socio-sanitary network, rates, and reimbursement methods for home care services. If necessary, direct the client to the person or structure that can advise them.- Ensure the link between IMAD and the private/public hospital sector to organize the return home of clients, with services provided by IMAD.- Promote the exchange of information useful and necessary for the client's care (according to the planned procedure).- Facilitate hospital discharges for clients and promote a return home under the best conditions.- Collaborate with the client, the caregiver, health professionals, and the social sector to prevent inappropriate hospitalizations.- Work in close collaboration with the team at the IMAD arcade located at the HES-SO.Profile- Bachelor of Science in Nursing or equivalent deemed and registered by the Swiss Red Cross.- Knowledge of the care process and clinical pathways, of the Geneva social health network.- Ability to work alone and in interprofessionality within a multidisciplinary team.- Sense of responsibility and solution-oriented, sense of prioritization of tasks and communication with others.- Ability to manage and resist stress and conflict.- Resistance to work.What we offerAt IMAD, we believe that a fulfilled professional is a high-performing professional.
