Join the medico-social centers of West Lausanne!We are actively looking for a Reference Nurse (%)Your missionAs a reference nurse, you ensure the role of manager of care. You carry out and delegate quality care to guarantee the health, safety, and well-being of clients at home. In their living environment, you define with them the solutions best adapted to their needs, thus developing a privileged relationship of trust. Discover more about this profession by watching the video below.Main responsibilities- Develop the care project according to the needs of clients and their entourage and provide the care provided for in the intervention plan.- Participate actively, in interdisciplinarity, in the evaluation and monitoring of care, manage the network, and collaborate closely with nursing and home help staff while ensuring the quality of care.- Perform medical-delegated, hygiene, and comfort care, while accompanying clients on a daily basis.- Receive, clarify, and orient requests for care, document follow-ups in the file according to institutional directives, thus guaranteeing the continuity of care.- Create and maintain lasting links with partners of the regional medico-social network (professionals, volunteers, user groups).- Ensure the role of reference person in the global follow-up of clients: advice, support in administrative and financial procedures.Profile- Bachelor of Science in Nursing or equivalent title (recognition by the Red Cross essential).- Good knowledge of the Swiss socio-sanitary field (first experience in Switzerland required).- Excellent analytical, synthesis, and communication skills.- Mastery of French (oral and written) and solid writing skills.- Ease in managing priorities and decision-making.- Mastery of team tools, sense of service, and ability to work both autonomously and in a team.We offer- Daytime hours mainly (7:30 AM–5:00 PM), with only one evening worked every two weeks until 10:00 PM and one weekend worked every eight weeks.- Working conditions and salary according to the collective labor agreement of the parapublic health sector.- Quality professional pension scheme, beyond legal minimums.- 5 weeks of vacation per year, then 6 weeks from age 50.- Perspectives for continuing education and evolution within the network.- Stimulating and humanly rich professional environment, serving the well-being of clients.Start date: Immediately or to be agreed upon.Interested?
