The ASPMAD, with its 9 CMS and 950 employees, takes care of clients at home annually and serves 105 municipalities in the North, is looking for a Reference Nurse %CMS of Yverdon-Ouest 80%CMS %Your missionsAs a reference nurse, your role involves an advanced mastery of specific skills:- Personalized care: Propose, organize, and coordinate intervention plans adapted to clients, ensuring their well-being and respect for their needs.- Rigorous follow-up: Supervise and regularly re-evaluate care, coordinating essential actions for the safety and continuity of care.- Documentary management: Responsible for the management of OPAS files and the updating of client records in accordance with guidelines.- Interdisciplinary collaboration: Work closely with the entire network (CMS team, caregivers, doctors, partners) to ensure comprehensive care.ProfileYou hold a HES nursing diploma or an equivalent title, reinforced by 2 years of experience, preferably in home care. You are versatile, attentive, and autonomous; possess excellent communication skills and show a pronounced interest in maintaining home care. Your ability to organize, to anticipate needs, and to adapt to any type of situation, as well as your competence in stress management, are essential qualities.
