We are looking for a close caregiver resource person at SIPeD. As a close caregiver resource person within the cantonal nursing team (SIPeD), you provide support to close caregivers to allow them to continue helping their loved ones under favorable conditions and according to their wishes. You also provide expertise in this area within the teams and offer individual support to close caregivers, enabling them to assess their needs, share experiences, or benefit from advice. You establish, in collaboration with the close caregiver and the person being cared for, an emergency card to anticipate the situation of the person being cared for in case of incapacity of the caregiver. You collaborate with interdisciplinary teams to identify the support needs of close caregivers and intervene in a coordinated manner with the close caregiver and the person being cared for. You train the CMS teams on close caregiver support and participate in the development of sessions. You hold an HES diploma (ideally social worker, nurse, or occupational therapist) or an equivalent qualification. Experience is a plus. You are a responsible and autonomous person. You are attentive, flexible, and adaptable to any situation. You have several years of experience, ideally in home care. You are comfortable with modern tools. You have a driving license.
