You will participate in the evolution of the home care and community health sector by responding with us to the needs of our population based on our values: TRANSPARENCY – EQUITY – RESPECT – TRUSTREFERENT NURSE AND CARE RESOURCE PERSON ON A PERMANENT CONTRACT AT 80%MissionAs a referent nurse (60%):- After a specific integration and training period regarding the diversity of situations and the organization of the CMS, you will be autonomous in the management of nursing care.- Accompanied by an expert multidisciplinary team, you will perform medico-technical care and guarantee the smooth running of their implementation by ensuring follow-up of our beneficiaries through updating their file and actively participating in meetings and discussions.- Privileging a global approach to the person, you will ensure prevention and health promotion for our beneficiaries and their families.- As a referent nurse, you will be a resource person within the team, coordinating interventions around the beneficiary and supporting the quality and continuity of care and adapting to new situations according to new guidelines.As a Care Resource Person (PRSP) (20%):- Promote and support a culture and practice of care aimed at offering the best possible quality of life to people suffering from progressive chronic diseases, regardless of age and stage of disease progression.- Inform/train on new developments related to care, particularly regarding processes, tools, and regulations.- Be an expert resource person for the members of interdisciplinary teams (HES, ASSC, auxiliaries) with the intention of developing skills.
