Liaison Nurse ARCADE / 80% / Permanent ContractIMAD is a pioneering and reference institution for home care in the canton of Geneva. It provides care, support, prevention, and health promotion services 7 days a week, 24 hours a day, to thousands of people of all ages, 25% of whom are under 65. It offers a flexible work environment that promotes a balance between professional and private life and conditions encouraging the development of skills as well as the sharing of knowledge and experience.Liaison Nurse ARCADE / 80% / Permanent ContractThe Liaison Nurse (IDL) ARCADE is responsible for informing, guiding, and registering requests from the public and in particular from future caregivers in order to ensure the maintenance of the quality of care. The IMAD arcade, located at the Gustave Julliard Building of the HUG, is at the service of the liaison with a presence 7 days a week. The IDL ARCADE works in close collaboration with the liaison nurses of the various liaison sites and ensures the relay of their activity on weekends and public holidays. The scope of the liaison nurse covers the hospitalized patient, requiring a return home with nursing care needs. Their field of activity is defined in particular in terms of needs assessment, medical validation of the required care, and provisional planning of interventions by home care teams.Missions and responsibilities- Inform the public and HUG staff about home care services offered by IMAD, partners of the Geneva socio-sanitary network, rates, and reimbursement methods for home care services.- As necessary, direct the user to the person or structure that can advise them.- Ensure telephone response for the caregiver information line.- Ensure the link between IMAD and the hospital sector to ensure the follow-up of hospitalized IMAD patients.- Ensure the link between the home care team and the hospital sector to organize the return home of patients requiring nursing care.- Ensure the registration of new IMAD patients.- Promote the exchange of information useful and necessary for patient care (according to the planned organization).- Facilitate hospital discharges for patients and promote a return home under the best conditions.- Contribute to health promotion and prevention by mastering the issues and the link with the Geneva network.- Collaborate with the patient, the caregiver, health and social professionals.- Prevent inappropriate care.- Work in close collaboration with the IMAD arcade team.Profile- Degree in nursing or equivalent deemed and registered by the Swiss Red Cross, with authorization to practice in the canton of Geneva.- Knowledge of the care process and clinical pathways, of the Geneva health and social network.- Ability to work alone and in interprofessionality within a multidisciplinary team.- Sense of responsibility and solution-oriented, sense of listening, communication, and empathy with others.- Ability to manage stress and priorities.What we offerAt IMAD, we believe that you are our greatest asset.
