NURSE for our service based in Estavayer-le-LacMain missions- Ensure the transition and continuity of care for clients, guests, and residents in the Broye fribourgeoise district.- Assess the needs of people with loss of autonomy at home or after a hospital stay.- Analyze requests for help and support, set up and coordinate the necessary services within 8 hours.- Collaborate closely with attending physicians, hospital teams, as well as social and health institutions in the network.- Contribute to the fluidity of care pathways and the optimization of health network resources.- Actively participate in projects.Profile sought- Nurse with a HES degree or equivalent recognized qualification.- 5 years of experience in home care or in the coordination of care or willingness to train.- Knowledge of the care network and the Swiss health system.- Excellent analytical, organizational, and communication skills in interdisciplinary work and in crisis management.- Mastery of common tools (patient file, request management).- Driver's license (travel in the Broye region).We offer- An open-ended contract within an interdisciplinary team that prioritizes human contact.- Varied work between the field, hospital, home, and institutions.- Social and salary conditions according to the Fribourg scale. jidddcd735aen jit0625aen jpiy26aen
