The Medical-Social Centers are tasked with helping individuals of all ages, dependent or with health issues, to remain in their living environment for as long as possible and to support family caregivers. The ASPMAD, with its 9 CMS X 950, takes care of clients at home annually and serves 105 municipalities in the North, is recruiting for our Orbe CMS, a mental health nurse % for a position as a mental health referent % Orbe. Your missions As a mental health nurse, your role as a resource person for your future team involves advanced mastery of specific skills: - Clinical: Conduct regular assessments to adapt care to the evolving and complex needs of clients. - Interprofessional: Work in synergy with a multidisciplinary care network to optimize care. - Leadership: Provide guidance and support to your colleagues, thanks to your role as a resource person in mental health, acting as a reference within your team. - Care planning and management: Organize personalized care plans in collaboration with clients while keeping client files updated in accordance with protocols, including profile sheets. You hold a nursing diploma or a recognized equivalent title, complemented by at least two years of experience with adults and/or a post-graduate diploma in mental health would represent an asset. You demonstrate autonomy, versatility, and a great capacity for listening, with a marked interest in home care and working with various stakeholders. You have the ability to easily adapt to different situations and you excel in communication. You possess a driver's license and, ideally, a vehicle.
