Social Worker – Field-based at 60%Pré Pariset brings together three medical-social establishments (EMS) located on 3 sites in the city of Pully, with a total of 132 beds. It employs 240 staff. Our mission is to support elderly people experiencing a loss of autonomy, in long and short-term care, as well as in our temporary reception center. We also manage a public restaurant, le Delta, with 54 seats. To complete the team and actively collaborate with the long-term care team, we are opening a position for a Social Worker – Field-based at 60%.Main missions- Coordinate and ensure activities within the temporary reception center and contribute to those of the long-term care unit, in close collaboration with the dedicated team, fostering social interactions between beneficiaries.- Welcome, listen to, and accompany beneficiaries throughout their journey, respecting their dignity and individuality.- Assess the needs of beneficiaries and guide them towards social resources (housing, health, etc.), with a perspective of maintaining them at home or, when the time comes, of orientation.- Implement individualized support projects in collaboration with members of the interdisciplinary team and the beneficiaries.- Collaborate with local partners (social services, institutions, CMS, etc.) to ensure comprehensive care for beneficiaries and their maintenance at home.- Support family caregivers through counseling and guidance actions.- Manage and follow up on beneficiary files.- Participate in the coordination of actions with multidisciplinary teams and contribute to professional meetings.- Guarantee respectful and benevolent support, in accordance with the values of the institution.- Organize and carry out work schedules for the team.Profile- HES/ES degree in social work or equivalent recognized title (sociocultural option preferred).- Confirmed experience in a similar function.- Excellent knowledge of the social network and aid mechanisms (social insurance, legislation, social assistance, etc.).- Great listening skills and ability to work autonomously while integrating into a team.- Sense of rigor and ability to maintain a benevolent attitude.- Mastery of computer tools (Microsoft Office, SADIES suite, etc.).- Ability to drive.We offer- A pleasant and motivating work environment.- An opportunity to join a small, dynamic team.- Possibilities for continuing education.- Various benefits (meals, etc.), according to Héviva scales.- Working conditions in compliance with the health CCT.Type of contract: Permanent or to be agreed upon.Activity rate: 60%Hierarchical superior: Director of CareYou can apply directly on our website under the jobs section.
