The CMS READOM program - Reference Nurse 80 - 100%The CMS READOM program in home care. The multidisciplinary team intervenes for a maximum of three weeks. It is composed of occupational therapists, nurses, nursing assistants (ASSC), physiotherapists, and health auxiliaries. It collaborates with the client, their entourage, and the care network. To complete our team, we are looking for a Reference Nurse READOM 80 - 100%.As a reference nurse, you contribute through your skills to the optimal quality of interventions with clients. You guarantee the link between the client, the multidisciplinary team, and partners. You respond to care needs by ensuring physical and mental health care. You collaborate in an interdisciplinary manner with the goal of comprehensive client care.You have a Bachelor of Science HES-SO in Nursing or a title deemed equivalent, with 3 years of experience in acute care. Experience in home care and knowledge of RAI HC would be assets. You are a responsible and autonomous person. You are attentive, flexible, and adapt to any situation.
