Management - Caregiver Resource Person - %We are looking for a Caregiver Resource Person at the SIPeD %As a caregiver resource person within the cantonal nursing team (SIPeD), you offer support to caregivers to enable them to continue helping their loved one under good conditions and according to their wishes. You also provide expertise in this area within the teams. You offer individual support to caregivers, allowing them to assess their needs, share their experiences, and benefit from advice. You establish, in collaboration with the caregiver and the person being cared for, an emergency card to anticipate the future of the person being cared for in the event of the caregiver's incapacity. You collaborate with interdisciplinary teams to identify the support needs of caregivers and intervene in a coordinated manner with the caregiver and the person being cared for. You train the CMS teams on the caregiver offer and participate in the development of projects during team meetings. You have a HES diploma (ideally social worker or nurse or occupational therapist) or an equivalent title. Experience in this field is an asset. You are a responsible and autonomous person. You are attentive, flexible, and you adapt to any situation. You have a few years of experience, ideally in home care. You are comfortable with computer tools. You have a driving license.
