Description

Welcoming 34 residents, the Fondation du Clos du Château (Corcelles-près-Concise) is a living space offering medico-social, hotel, care and activity services to people experiencing loss of autonomy for whom staying at home is no longer sustainable or temporarily possible. The institution also includes an on-site sheltered workshop and an off-site sheltered workshop (CAT Au Fil du Talent) in Chavornay.

To complete our care team, we are looking for a:

NURSE at 80%

Your mission:

determine and meet the residents’ needs from a physical, psychological, social and spiritual perspective determine and plan the required care develop individualised care and support plans for residents provide, supervise and monitor care services and delegated medical acts ensure continuity of care by ensuring daily information transfer lead family and network meetings organise the daily tasks of the care team take on shifts according to the established schedule

Your profile:

you hold a nursing diploma (Bachelor of Science HES-SO, ES diploma or equivalent recognised by the Swiss Red Cross) you are sensitive to all aspects of elderly care you have a collaborative spirit and ease in interdisciplinary work you are proficient with common IT tools you live within a maximum radius of 35 km
