Nurse (80–100%)Proxi-Soins is a home care provider active in the cantons of Vaud and Fribourg. To strengthen our team, we are looking for a Nurse at 80–100% for our site. You will occupy a central role in care, team supervision, and the development of care quality, working in close collaboration with the care management.Care missions- Ensure the follow-up of new patients and their care.- Supervise care files and guarantee their compliance.- Organize and adjust field team schedules.- Ensure the continuity and quality of care provided to patients.- Coordinate with doctors, hospitals, and nursing homes in the health network.Team management- Supervise and support nurses, nursing assistants, auxiliaries, and staff.- Participate in recruitment, integration, and follow-up of staff.- Organize replacements, manage absences and vacations.- Participate in the development of team skills through coaching and internal training.Administrative management- Manage procedures related to care billing.- Ensure the follow-up of OPAS and documents necessary for care coverage.- Guarantee the quality and reliability of data entered in patient files.Quality and development- Participate in the continuous improvement of care.- Participate in updating internal procedures and protocols.- Participate in the development of the structure and its reputation within the health network.- Participate in telephone on-call duties according to a rotation schedule.- Manage urgent situations and provide support to teams during on-call periods.
