For our CMS X we are looking for: Mental Health Nurse Referent at 60% - CMS X: 60% Your mission As a mental health nurse referent, you propose, organize, and coordinate the intervention plan with the client, ensuring their needs and requests are respected. You guarantee the support and follow-up of mental health clients in their entirety. You ensure the necessary actions are taken for the safety and continuity of care. You possess a good level of technical skills and are also comfortable in caring for clients. You manage the OPAS forms process and keep the client file updated according to established guidelines. You collaborate with the entire network (CMS team, caregivers, doctors, partners) to ensure optimal quality of care for the profile. You hold a nursing HES degree or an equivalent qualification. Autonomous, versatile, and attentive to others, you show a clear interest in home care and interdisciplinary work. You can easily adapt to any type of situation, and problem-solving is one of your strengths. You have a good knowledge of home care and solid experience in mental health.
