The Saphir Foundation, a multi-mission foundation (psychogeriatrics, geriatrics, adult psychiatry), is active in the medico-social field in the canton of Vaud and in the reception and support of people weakened in their physical and psychological integrity during their life process.

Within this framework and to complete our team, we are looking for our OSAD / Home Care Organisation in Yverdon-les-Bains, which serves 177 beneficiaries and is aimed at adults with mental and/or psychological disorders whose life path requires home support:



Mental Health Nurse on a Permanent Contract at 80%-100% OSAD



Whatever their role, the professionals at the Saphir Foundation share a common goal: to support individuals daily to enable them to concretely experience the institutional service strategy "like at home". This means promoting the maintenance of their habits, offering a response adapted to their wishes and, through genuine interprofessional collaboration, ensuring the well-being and happiness of the individuals.

As a mental health nurse, you design, deliver and evaluate preventive, therapeutic and rehabilitation nursing care services, in partnership with the individuals. You build a professional relationship with them within the perspective of their care plan. You actively promote their overall health as well as their support in managing their health process.

Your presence in the field is essential: your interventions are based on observation, the transmission of best practices and the accountability of your mandate. Thanks to your expertise, you are able to reflectively assess the physical, psychological, social and spiritual needs of the individuals, and analyse situations with a systemic approach. You participate in examinations and observations related to medical diagnosis, manage emergencies and activate emergency procedures when necessary.

In line with your person-centred values, you establish a lasting relationship of trust with the individuals and their families, co-constructing a safe, respectful and adapted environment to their needs.

YOUR BACKGROUND - YOUR QUALITIES AND SKILLS

  • You hold a Bachelor’s degree in nursing or a recognised equivalent, complemented by solid experience in adult psychiatry.
  • You possess a strong sense of observation, enabling you to quickly assess complex situations.
  • You work based on evidence, with reflective practice and a thorough clinical analysis capacity.
  • You are familiar with the recovery concept, which you apply daily in your support.
  • You have proven knowledge of the social field and the Swiss health system; a minimum of 5 years’ experience in outpatient care is an asset.
  • You place the individual’s well-being at the centre of your concerns, using appropriate relational and therapeutic tools.
  • Your natural leadership, sense of responsibility, autonomy, and ability to self-reflect allow you to guarantee the quality of the expected services.
  • You are recognised for your empathy, listening skills, flexibility, adaptability and organisation in a demanding interprofessional environment.
  • You understand that stress, effective communication and priority management are integral to the profession and you handle them with mastery.
  • You have perfect command of spoken and written French, and regularly use IT tools (notably Carefolio).
  • A driving licence and a personal vehicle are essential.
