As a member of the PCLD, you will be required to intervene throughout the Canton of Geneva for long-term replacements, ranging from 6 to 15 months, within our branches. Your main mission will be to ensure continuity of care for patients by acting as a reference in managing their files and providing quality care.We are looking for committed individuals to fill a position where you will have the opportunity to:- Take on stimulating challenges and gain enriching professional experiences.- Broaden your horizons by working for a long duration in different sectors in Geneva, within various teams and with different types of cases, gaining in-depth knowledge of the healthcare and social network in Geneva, as well as of our institution and the field of home care and assistance.- Develop your theoretical and practical skills through tailored training.By joining the PCLD team, you will be able to:- Assume the role of a reference for patients’ needs according to the ethical principles of the nursing profession, within the home care team model.- Provide personalized, autonomous, or medically delegated nursing care, covering a range of preventive services, directly at the patients’ homes, for individuals of all ages.- Work closely with community health and care professionals as well as multidisciplinary assistants, delegating care according to established procedures.- Organize medical-social procedures and ensure coordination with all stakeholders involved in the care of a particular patient, mobilizing the available resources of the primary network.We require a Bachelor HES-SO in nursing or an equivalent qualification, as well as recognition of excellent capability and proven flexibility and availability, a sense of responsibility, and problem-solving skills essential for facing challenges. You should have the ability to work independently and in interprofessional environments within a team.
