Home-based Reference Nurse at 80 or 100% on a permanent contract within the centralised team for long-term replacement assignments (6 to 15 months) with responsibility for a portfolio of patient files

As a member of the PCLD, you will be called upon to work throughout the Canton of Geneva for long-term replacements, ranging from 6 to 15 months, within our branches. Your main mission will be to ensure continuity of patient follow-up, acting as a reference in managing their files and providing quality services.

By joining the PCLD team, you will be able to:

  • Assume the role of reference in assessing patients’ needs, in accordance with the ethical principles of the nursing profession, within the home care team model.
  • Provide personalised nursing care, autonomously or under medical delegation, covering a full range of preventive, curative, educational, and palliative services, directly at patients’ homes of all ages.
  • Work closely with community care assistants and multidisciplinary aides, delegating care according to established procedures.
  • Organise medico-social procedures and ensure coordination with all stakeholders involved in a given patient’s situation, mobilising available resources from the primary and secondary network.

Qualifications

  • A HES-SO Bachelor’s degree in nursing or an equivalent qualification, as well as recognition from the Red Cross.
  • Excellent adaptability and delegation skills, with demonstrated flexibility and availability.
  • A sense of responsibility and solution-oriented approach, essential to meet encountered challenges.
  • Ability to work autonomously and interprofessionally within a multidisciplinary team.
  • Essential mobility, which may involve travelling on foot, by bicycle, public transport (TPG), or car depending on assignments.
