To fulfill this mission, Maisonneuve has set up an ecosystem to take charge of the patient and accompany them. We operate through three main structures: The Clinique Châtelaine for post-acute care, specializing in musculoskeletal, internal medicine, oncology, and geriatrics. The Charmilles, a Home Care center, intended for patients of the Clinique Châtelaine, to ensure care at their home. Recently, two new structures opened their doors in Belle-Terre, Thônex, composed of a clinic, Clinique Belle-Terre, and a care center. As part of the growth of our structure, we are looking for the Clinique for a:Care ManagerYour missionAs Care Manager, you ensure the steering, organization, and coordination of care activities to guarantee quality, secure, and patient-centered care. You ensure the continuity of care pathways, support nursing teams, and actively contribute to the development of a culture of excellence, quality, and safety within the establishment.Steering and coordination of care- You coordinate care pathways, from admission to patient discharge, ensuring their fluidity, continuity, and efficiency.- You promote close collaboration between nursing teams, medical staff, therapists, and network partners to ensure quality interdisciplinary care and a positive patient experience.Quality and safety of care- You organize service activities to ensure efficient care, compliant with standards and regulatory requirements.- You guarantee procedures and protocols, ensure the monitoring of quality and safety indicators, participate in continuous improvement processes, and collaborate closely with the Quality Department for the deployment of institutional projects.Team management- You supervise, support, and develop nursing teams by fostering a caring and empowering work environment.- You ensure the management of resources, support change, sustain skills development, and ensure the maintenance of team dynamics favoring the quality of care.Training and development- You supervise the reception, integration, and supervision of new students and interns.- You identify training needs, encourage skills development, and contribute to the dissemination of good professional practices within the establishment.Your profile- HES Bachelor in Nursing or equivalent diploma.- Training in management or willingness to obtain it.- Confirmed experience in team management, ideally in a hospital or clinical setting.- Knowledge of care pathways, quality processes, and Swiss regulatory requirements.- Leadership, sense of organization, team spirit, and ability to support change.Advantages of joining us- A human work environment centered on the patient.- A position with responsibilities with a key role in the development of the establishment.- Opportunities for continuing education and professional development.- An interdisciplinary environment favoring exchange and continuous improvement. jid2c3ba3caen jit0832aen jpiy26aen
