IMAD is a pioneering and reference institution in home care in the canton of Geneva. It provides care, support, prevention, and health promotion services 7 days a week, 24 hours a day, to more than 18,000 patients of all ages, 25% of whom are under 65 years old.
With its 2,300 expert and committed collaborators, including more than 50 teams deployed throughout the canton, IMAD's mission is to meet the population's needs and their wish to remain at home autonomously while preserving their quality of life during periods of temporary or prolonged fragility. It also helps to limit hospital stays, facilitate return home, and relieve informal carers.

Liaison Nurse / 100% / 7-month Fixed-term Contract
The specific scope of the liaison nurse covers IMAD clients (known or future clients) hospitalised who require a return home with IMAD services. Their activity includes assessing needs, medical validation of required services, and coordinating planned interventions by home care teams (EMD) and/or centralised services.
Your missions and responsibilities
- Inform clients or prospective clients about home care services offered by IMAD, partners in the Geneva socio-health network, tariffs, and reimbursement modalities for home care services. When necessary, direct the client to the person or structure that can advise them.
- Ensure coordination between IMAD and the private/public hospital sector to organise clients’ return home with services provided by IMAD.
- Promote the exchange of useful and necessary information for client care (according to the required documentation).
- Facilitate clients’ hospital discharges and promote return home under the best conditions.
- Collaborate with the client, informal carer, health and social professionals.
- Prevent inappropriate (re)hospitalisations.
- Work closely with the IMAD arcade team located at the HUG.
Qualifications
- Bachelor HES-SO in nursing or equivalent training recognised and registered by the Swiss Red Cross
- Knowledge of care processes and clinical pathways, Geneva health and social network
- Ability to work independently and interprofessionally within a multidisciplinary team
- Sense of responsibility and solution orientation, communication skills, negotiation, and interpersonal relations
- Adaptability and delegation skills
- Stress and conflict management
- Physical work endurance.
