Description

For our coordination service based in Estavayer-le-Lac:

Main missions

• Ensure coordination, transition and continuity of care for clients, hosts, residents in the Broye district of Fribourg.
• Assess the needs of people losing autonomy at home or after a hospital stay (HIB, HFR)
• Analyse requests for aid and care services.
• Implement and coordinate the necessary services within 8 hours.
• Work closely with attending physicians, hospital teams, as well as social and health institutions in the network.
• Contribute to the smoothness of care pathways and optimisation of health network resources.
• Actively participate in institutional projects.

Desired profile

• HES qualified nurse or equivalent training.
• 5 years experience in home care or care coordination.
• CAS in care coordination or willingness to train.
• Knowledge of the Swiss health care network and system.
• Excellent analytical, organisational and communication skills.
• Comfortable with interdisciplinary and networked work.
• Proficient with common IT tools (electronic patient record, request management).
• Driving licence (travel within the Broye).
