With nearly 1,500 employees, the Department of Rehabilitation and Geriatrics (DRG) is a medical centre focused on the elderly and is spread across 5 sites: Trois-Chêne, Loëx, Bellerive, Beau-Séjour and Joli-Mont. It manages acute situations, notably through the emergency services at the Trois-Chêne Hospital, age-specific issues, medical rehabilitation, and palliative medicine.

The Palliative Care and Supportive Care Centre at the HUG is an interdepartmental medical structure that brings together the varied expertise essential for the care of patients requiring a palliative approach. It develops and coordinates the optimal strategy to integrate this palliative approach into the care provided to patients outside specialised palliative care units.



Case Manager - Palliative Care and Supportive Care Centre - 80%


Reporting to the head of the palliative care service, you supervise multidisciplinary care in accordance with the procedures established by the Centre.

You coordinate the network of palliative care referents in collaboration with the departments as well as the various activities of the centre (organisation of symposiums, RESOPAL).

You collaborate with the Patient Journey Manager (IPM) and the RES to improve care. You participate in the coordination of ongoing projects.

You train the relevant employees in identifying patients eligible for standardised or complex care, as well as in the use of assessment and monitoring tools related to processes and standardised care.

You implement and monitor relevant indicators (e.g., coverage rate of care).

You improve the quality of care by proposing quality actions and participate in promoting the Centre within the network.


Qualifications

You hold a HES qualification, complemented by a specialisation in the Centre's field. Skills in project management and/or quality of care, certified by training, are highly desirable.

Professional experience in a palliative care service combined with a certificate in palliative care (certificate or diploma in palliative care) is an asset.

You have expertise in process coordination. You demonstrate strong analytical and synthesis skills, as well as the ability to work within a network. You are methodical, rigorous, and show a critical and innovative mindset.

You possess interpersonal skills and a strong sense of leadership. You demonstrate great adaptability and flexibility.

You have a solid command of IT tools, both internal and external. A thorough knowledge of the HUG is an additional asset.
