Rooted in an approach based on the concept of recovery and the development of individual strengths, we accompany each person in the construction of their life project, by promoting autonomy, active participation, and the mobilization of each individual's own resources.To pursue this mission with high standards and benevolence, we are looking for, for an open-ended position, a Social Worker in adult psychiatry.Start date: As soon as possible or by agreementWorkplace: LausanneYour mission will be to:- Manage placement requests from the CCICp, hospitals, and other institutional partners- Submit placement requests to the managers of the host establishments- Organize pre-admission or transfer networks in collaboration with the partners concerned- Accompany users in their procedures (insurance, social benefits, relations with private individuals and the services of the Justice of the Peace and professional guardianships)- Advise support teams on aspects related to interprofessional networks as a contact person for extraordinary funding requests- Develop, in collaboration with support teams, social and/or professional integration projects- Support the implementation of individualized reintegration projects in close connection with follow-up professionals- Ensure communication with the various bodies and partners involved- Intervene occasionally at our mission-based structures within the framework of specific mandatesYour profile:- You hold a degree in social work, with several years of experience in the field of adult psychiatry in Switzerland- You have excellent knowledge of the Vaud socio-sanitary system- You master the Swiss social security system- You are comfortable with file management- You possess solid analytical and synthesis skills- You demonstrate a good sense of organization and appreciate teamwork- You are comfortable with current IT tools- You appreciate working in a multidisciplinary team- You hold a driver's licenseWe offer:- A position with dynamic challenges- A training program- Working conditions according to the collective labor agreement of the Vaud parapublic health sector- Discounts in more than 150 storesAdditional information:We will be happy to receive your complete files (cover letter – CV – work certificates – diplomas) by email.
