We are looking for the CMS of Moudon, for a

Mission:

• Collaborate on the medico-social care project (support in the form of assistance and care) based on respecting the principle of personal autonomy.
• Participate in monitoring the client’s situation evolution, in collaboration with the client, their circle, the interdisciplinary team and the attending physicians.
• Provide clients with services such as basic care, nursing care, household help and family assistance.

Profile:

• Hold a Federal VET Diploma as ASSC.
• Have, if possible, experience in home care.
• Be available to work during the day, evenings and weekends.
• Be able to work independently and in an interdisciplinary manner.
• Hold a driving licence and own a vehicle (possibility to use a company vehicle).
