To complete our teams, we are recruiting: A Community Care and Health AssistantMain ActivitiesYour activities and responsibilities will include:- Coordinating and communicating with various functional teams- Participating in the management of complex symptoms in acute phase patients- Supporting and accompanying families and relatives of patients at the end of life, focusing on overall well-being and quality of life- Promoting and developing interdisciplinary cooperation- Performing medical-technical acts under the responsibility of nursing staff- Ensuring the quality of care within a bio-psycho-social dimension, addressing patients' needs and habits appropriately, prioritizing autonomy, and preventing dangers- Participating in actions in crisis and end-of-life situations- Committing to training and staying informed about innovations in your professional activityProfileRequirements are:- Holding a Federal Certificate of Capacity as an Assistant in Community Care- Experience in acute care or care settings, ideally at least 1 year- Mastery of the French language- Proficiency with tools (Microsoft Office, Soarian…)What we offer- A pleasant, stimulating, and caring work environment- A dynamic and supportive team- Support in development and human values centered around the needs of our patients and their relatives, as well as the well-being of patientsEntry: as soon as possibleInterested?
