The Health Network of the Lausanne Region is an association that brings together more than a hundred institutions and health professionals - CHUV, treatment centers and nursing homes, home care (CMS), and doctors - as well as approximately 95 professionals contributing to continuity of care, particularly through the Regional Office and (BRIO). We also undertake projects aimed at improving the quality of care, mainly benefiting elderly individuals who are frail due to one or more chronic illnesses. To complete our team, we are looking for a: Liaison Nurse. You enjoy working independently while creating links? You are sensitive to supporting frail elderly persons and their families? You wish to engage in a dynamic activity by working with and in different care settings? Then you are the person we are looking for!Your mission:Anticipate and plan the user’s care within the care network according to their needs in collaboration with other professionals while taking into account their needs and the resources of the system. Organize both short and long stays in nursing homes and the return to home with the necessary assistance and care. Facilitate and coordinate continuity of care by communicating and acting as an interface/pivot between various partners. Inform and propose to the user, their caregivers, and professionals the perspectives based on the evolution of the situation.Profile requirements:Nursing diploma (Bachelor HES or equivalent). Professional experience in hospital, community settings, and good knowledge of the Vaud socio-health network. Ability to work with family, medical, and institutional networks in an innovative context, having strong listening skills, and proficiency in common office tools as well as ease in analysis, synthesis, and prioritization management.We offer you:A varied role in a public interest field that is constantly evolving within a motivated and dynamic team.
